A Homeless Healthcare Collaborative operates within a street medicine mobile clinic program, aiding patients in accessing community services and adopting healthy behaviors. They provide integral support to providers and team members, conducting care management and outreach in various locations, including homeless encampments, shelters, and health fairs.

A Caregiver provides support to individuals who face challenges in completing fundamental tasks due to factors such as age, mental health issues, or disabilities. Responsibilities of a Caregiver encompass tasks like administering medications, aiding clients with personal care activities such as bathing, dressing, eating, and grooming, and adhering to the patient's designated healthcare regimen.

I am so thankful our organization chose Minnesota West Community & Technical College for my CHW certificate. The staff went above and beyond, providing skills and information specific to my education. Despite having a bachelor's in social work, I found this program more applicable to my work as a CHW. My professors were also understanding of my full-time job and helped me complete the program.
